RESUMO

Objetivos: Refletir sobre as desigualdades que afetam a Enfermagem em sua trajetória histórica e que se acentuam durante a pandemia da Covid-19. Métodos: Método crítico-reflexivo com aporte no referencial do materialismo histórico-dialético. Foram analisados posicionamentos das entidades de classe da Enfermagem, boletins epidemiológicos e notícias sobre o contexto de trabalho de profissionais da saúde, desde março de 2020. Resultados: Foram discutidas questões acerca das desigualdades e hierarquias próprias das equipes de saúde, tendo em vista as perspectivas de classe, gênero, raça/etnia que impactam na profissão de enfermagem e no trabalho em saúde e que se acentuam no contexto da COVID-19. Os impactos da pandemia expõem a desvalorização do trabalho da enfermagem, evidenciada pela invisibilidade social da categoria e pela precarização da vida de quem a exerce. Conclusão e implicação para a prática: Torna-se urgente e necessário reconhecer que as desigualdades agravadas pela pandemia fazem parte de uma condição estrutural da sociedade que afeta diretamente trabalhadoras e trabalhadores da Enfermagem. Ademais, torna-se oportuno a enfermagem perfilhar lutas junto a sociedade civil na defesa igualitarista de justiça e pela proteção social universal e na superação dos condutores estruturais das desigualdades


Objectives: to reflect on the inequalities that affect Nursing in its historical trajectory and that are accentuated during the COVID-19 pandemic. Methods: critical-reflective method based on historical-dialectical materialism. Positions of Nursing class entities, epidemiological bulletins and news about the work context of health professionals were analyzed since March 2020. Results: issues about the inequalities and hierarchies specific to health teams were discussed, in view of the perspectives of class, gender and race/ethnicity that exert an impact on the Nursing profession and health work and that are accentuated in the context of COVID-19. The impacts of the pandemic expose the devaluation of the Nursing work, evidenced by the social invisibility of the category and the precariousness of the life of those who exercise it. Conclusion and implication for the practice: It is urgent and necessary to recognize that the inequalities aggravated by the pandemic are part of a structural condition of society, and that it directly affects Nursing workers. Furthermore, it is opportune for Nursing to profile struggles with civil society in the egalitarian defense of justice, for universal social protection and in overcoming the structural drivers of inequalities


Objetivos: Reflexionar sobre las desigualdades que afectan a la Enfermería en su trayectoria histórica y que se acentúan durante la pandemia Covid-19. Métodos: Método crítico-reflexivo basado en el materialismo histórico-dialéctico. Se analizaron posiciones de entidades de clase de enfermería, boletines epidemiológicos y noticias sobre el contexto laboral de los profesionales de la salud desde marzo de 2020. Resultados: Se discutieron cuestiones sobre las desigualdades y jerarquías propias de los equipos de salud, en vista de las perspectivas de clase, género, raza / etnia que impactan la profesión de enfermería y el trabajo en salud y que se acentúan en el contexto del COVID-19. Los impactos de la pandemia exponen la desvalorización del trabajo de enfermería, evidenciada por la invisibilidad social de la categoría y la precariedad de la vida de quienes la ejercen. Conclusión e implicación para la práctica: Es urgente y necesario reconocer que las desigualdades agravadas por la pandemia forman parte de una condición estructural de la sociedad y afectan directamente a los trabajadores de enfermería. Además, es oportuno que la enfermería perfile las luchas con la sociedad civil en la defensa de la igualdad y la justicia, por la protección social universal y la superación de los impulsores estructurales de las desigualdades

Resumo Objetivo refletir sobre a figura pública de Florence Nightingale, suas realizações, Reforma Sanitária e a criação da Escola de Enfermeiras, e compreender o nascimento da enfermagem como profissão. Método partiu-se da literatura de um quadro das pressões sociais que agiam sobre o comportamento individual de Florence Nightingale e dos marcos divisórios aparentes, que entendemos como a densidade das relações sócio-históricas, e o seu tempo social. Análise sócio-histórica da história de vida de Florence Nightingale e da literatura social de Charles Dickens. O marco temporal compreendeu da promulgação da New Poor Law (1.834) à revogação (1.601). Resultados Florence Nightingale foi uma mulher adiante do seu tempo que, contrariando as teorias do Darwinismo social de sua época, criou a profissão da enfermeira, e produziu uma clivagem na profissão definindo-a como ciência e arte. Conclusão e implicações para a enfermagem ao criar a figura emblemática da Dama da Lâmpada, Florence Nightingale gravou no cuidado de enfermagem, o zelo, o desvelo e a compaixão, aqui entendida como empatia e piedade com o sofrimento do outro acompanhada do desejo de minorá-lo, uma participação espiritual na dor do outro.


Resumen Objetivo reflexionar sobre la figura pública de Florence Nightingale, sus logros, Reforma Sanitaria y la creación de la Escuela de Enfermeras, y comprender el nacimiento de la enfermería como profesión. Método se partió de la literatura de un cuadro de las presiones sociales sobre el comportamiento individual de Florence Nightingale y de los marcos divisorios aparentes que se entiende como la densidad de las relaciones socio histórico y su tiempo social. Análisis socio histórico de la historia de vida de Florence Nightingale y de la literatura social de Charles Dickens. El marco temporal se comprendió entre la promulgación del New Poor Law en 1834 y su revocación promulgada en 1601. Resultados Florence Nightingale fue una mujer adelante a su tiempo que, contrariando las teorías del Darwinismo social de su época, creó la profesión de enfermera, y produjo una mirada embrionaria en la profesión definiéndola como ciencia y arte. Conclusión e implicaciones para la enfermería al crear la figura emblemática de la Dama de la Lámpara, Florence Nightingale registró en el cuidado de enfermería, el celo, el cuidado y la compasión, entendido aquí como empatía y piedad con el sufrimiento del otro acompañado del deseo de una disminución, una participación espiritual en el dolor del otro.


Abstract Objective to reflect on Florence Nightingale's public figure, her achievements, Health Care Reform and the creation of the School for Nurses, and understand the birth of nursing as a profession. Method a framework of the social pressures acting on Florence Nightingale's individual behavior and the apparent dividing marks, which we understand as the density of socio-historical relations, and her social time, was drawn from the literature. This is a socio-historical analysis of Florence Nightingale's life story and Charles Dickens' social literature. The time frame spanned from the enactment of the New Poor Law (1834) to the repeal (1601). Results Florence Nightingale was a woman ahead of her time who, going against the theories of social Darwinism of her time, created the nurse profession, and produced a divide in the profession by defining it as science and art. Conclusion and implications for nursing by creating the emblematic figure of the Lady of the Lamp, Florence Nightingale engraved in nursing care, zeal, devotion, and compassion, here understood as empathy and pity for the suffering of others accompanied by the desire to alleviate it, a spiritual participation in the pain of others.

RESUMO

The year 2020 is an historic year on many accounts. It was to be an international year of celebration for nurses and midwives but turned into a year of turmoil with the emergence of the coronavirus pandemic and a call for justice and social awareness. Nurses were in the forefront and demonstrated courage and heroic efforts in the face of devastating events. Advanced practice nurses need to document their stories. This becomes our legacy. In an effort to preserve our legacy, the American Association of Nurse Practitioners history committee is in the process of identifying the existing historical documents for preservation.

Historians tell us there are perils to not knowing history and that studying history allows us to understand our past as well as our present. The how and why of our nurse practitioner (NP) role today lies in our history. The NP role was created in the 1960s by Dr. Loretta Ford and Dr. Henry Silver in Colorado out of need to increase patients' access to pediatric care. Today, Dr. Ford believes NPs providing high-quality primary care can solve what ails the country: access to care. To better understand how the NP role developed, what pushback was met from health care professionals and patients that hindered practice and how this was resolved, how NP practice has changed, and pearls of wisdom for contemporary NPs were questions asked of six pioneer NPs (female, n = 5; male, n = 1). Their collated responses illuminate myriad ways the role was established and how each one overcame restrictions and barriers to practice. These pioneer NP participants highlighted ways NP practice has changed since those early days. They imparted a variety of pearls of wisdom that can guide contemporary NPs to address scope of practice barriers and overcome patient's and other health professionals' objections to NP practice. Loretta Ford cautions us to remember the most important word in NP is "Nurse" and we need to be mindful of our professional roots. Many of these pioneer respondents noted the importance of collective voices when dealing with professional and practice issues; they urge all NPs to join and be active in professional organizations that are invested in promoting and protecting the NP role.

The nurse practitioner (NP) role has existed for 50 years. During the past 10 years, a national effort to use NPs to the full extent of their education based on the 2008 Consensus Model for Advanced Practice Registered Nurse Regulation and the 2010 Institute of Medicine Report on the Future of Nursing continues to result in variable scopes of practice (SOP) between states. Subsequently, NPs have a lack of clarity on SOP because it relates to population foci and practice setting. Review and analysis of state-based statutes and rules with the current literature focused on NP SOP, including documents by the National Council of State Boards of Nursing, was conducted. Clarification and understanding of SOP is essential for safe practice and optimum access to care. Inconsistent SOP regulation continues to exist between states, and NPs can be in employment situations that place them in a position to possibly breech their SOP. Although practice is not setting specific, NP licensure is based on NP education and certification that is competency based within population foci, and credentialing by employers should align with these parameters. Continuing to work toward a more common NP SOP between states and achievement of full practice authority means periodic reassessment of NP education and practice models. Practice mobility is essential for continued advancement of the NP profession and increased access to care by the public.

In 1928 members of the South Dakota State Medical Association (SDSMA or the Association) held a special meeting in Huron to consider a basic science bill that conformed "…in its entirety to the conditions existing in our state." Their draft bill proposed a standardized examination for all practitioners of the healing arts. A legislative committee, with its attorney, "…was in Pierre during the early part of the 1929 legislative session to make sure the bill was properly launched and in effective channels." Shortly after its introduction, the bill was withdrawn due to opposition from one SDSMA district whose legislative representatives were among the most influential in the legislature. A similar bill promoted by the SDSMA in 1933 also failed. It would be another six years before a basic science bill was enacted by the legislature. Eighty-nine years later, a bill governing the practice of certified nurse practitioners (NP) and certified nurse midwives (NM), including a board independent of the South Dakota Board of Medical and Osteopathic Examiners, was considered (Senate Bill 61). Introduced by a senator who characterized herself as representing the "House of Nursing," the bill challenged "…the overarching role that medicine thinks and perceives that they may have regarding advanced practice nursing practice." SB 61 passed in the senate and house and was signed by the governor. For this legislation in the 1930s and in 2017, the SDSMA's interest was defining and maintaining control of medical practice under the twin rubrics of quality and patient welfare. In both circumstances, legislators and other health care professional organizations contested not only the SDSMA's motivations, but also the evidence supporting their efforts. Our research explored (1) whether the collective viewpoints and conduct of the legislature, the SDSMA, and non-physician medical professionals are comparable in the two circumstances; and (2) if the circumstances are comparable, can we derive a useful concept or theme that could help guide the SDSMA in the future?

RESUMO

The role of the Emergency Nurse Practitioner (ENP) has evolved as a specialty and is appreciated within the context of the 2008 Consensus Model document (APRN Consensus Work Group & National Council of State Boards of Nursing APRN Advisory Committee, 2008). The first in a series of five articles, this article describes the appraisal of the ENP role as well as the specialty and the distinctive role of the ENP. The second article, Emergency Nurse Practitioner Practice Analysis: Report and Implications of the Findings, presents research to support the scientific basis of emergency specialty practice and content validity for a national certification program. Article 3, Beyond Competencies; Practice Standards for Emergency Nurse Practitioners: A Model for Clinicians, Educators, and Employers, introduces a new conceptual model that defines the specialty of emergency care's knowledge, skills, and abilities identified by the ENP practice analysis as practice standards not traditional competencies. In Article 4, Proposed Standardized Educational Preparation for the Emergency Nurse Practitioner, essential content for ENP preparation within graduate, postgraduate, and doctoral programs. The fifth article, Envisioning the Future for ENPs: Implications for Clinical Practice, Education, Research, and Health Policy, describes how ENPs are envisioning and impacting the future of emergency care and how their evolution can serve as a model for development of other advanced practice nursing specialties.

BACKGROUND: Australian legislation supporting the nurse practitioner (NP) role was enacted in 1998. Since then, NPs have played an important advanced practice role within the interdisciplinary healthcare team. However, the literature suggests that transition to the NP role can be challenging. AIM: This paper highlights the complex transition experiences of ten recently endorsed Australian NPs. The convoluted legislative and regulatory requirements that were negotiated by the NPs are presented as narratives. METHODS: Informed by an ethnographic approach, participants were interviewed several times during their first year. Interview transcripts were thematically analysed and aggregated into three narratives representative of key findings. KEY FINDINGS: The findings exemplify the complexity of navigating through a labyrinth of bureaucracy and the extensive negotiations required to appease those who yielded power over their future practice. CONCLUSION: This study raises awareness of the transition experiences of Australian NPs and their challenges and barriers during this time.
